Des Moines police officials determined that all three officers driving in two April crashes involving squad cars were at fault in the collisions and said all three have been disciplined.
The first wreck occurred April 7 at the intersection of East 12th Street and Washington Avenue, which has a four-way stop sign.
One car was driven by Officer Phil Terrones, who has been with Des Moines police since 1989. The other was driven by Officer Brandon Holtan, who graduated from the police academy in January and was in his final week of field training at the time. Both were headed to a report of shots fired..
